The "modify details" form has to be accessed via a newsletter or autoresponder. eMarket Solutions has to load up the subscribers information and this can't be done if the form is on your website.

Once you have created your modify details form under the forms tab in the top right hand side of the email software, edit your newsletter or autoresponder and create some text or an image like "modify your subsription" highlight this text or image and select the link button in the editor and under the built in links drop down menu select the modify form.
